Reporting Lost or Stolen Devices

Below are the steps you must take to report a lost or stolen device.

File a Police Report

It is your responsibility to file a police report in the jurisdiction where the device was lost or stolen as soon as possible.

If the device was lost or stolen on UTMB property contact the UTMB Police; otherwise contact your local police department.

Document the following information regarding your police report:

  • The date and time the device was lost or stolen
  • The approximate address where the device was lost or stolen
  • The name of the State and City in which the device was lost or stolen
  • The name of the responding police department
  • The police report number

If a local police department was contacted, contact the UTMB Police and file a second police report with them, providing them with the details of the first police report. If UTMB Police provides you with another case number, document that as well.

Notify the School of Nursing Multimedia Lab

Email the SON Multimedia Lab at SONMultimediaLab@utmb.edu as soon as possible with the police report information collected above.

Notify the UTMB Information Security Office

Notify the UTMB Information Security Office at CIRT@utmb.edu and copy (CC) the SON Multimedia Lab at SONMultimediaLab@utmb.edu immediately with the police report information collected above.

Please remember to copy (CC) the SON Multimedia Lab so that additional information can be provided to the UTMB Information Security Office as needed.
